The Prime Directive is evil, but its a necessary evil. Otherwise you end up on a slippery slope to the Federation playing the current USA, trying to police the entirety of world affairs (and inevitably making shit worse). As pointed out by the above, your example doesn't work because saving a single child is statistically impossible to affect the outcome of a civilization. Although, outside the fourth wall, I feel as if the PD was invented as a variation to naturalists who study wild animals and will not interfere, letting "nature take its course". However, this flies completely in the face of the fact that humanity will also (however arbitrarily) select species to attempt to bring back from the brink of extinction. Sometimes they even justify this by saying the cause of this is human activity, like encroachment on their habitat, or hunting, or pollution, or whatever else. However, most will recognize that it's usually only done for political or monetary reasons. Cute species are saved because they bring people to zoos, and iconic ones (giant pandas for China, bald eagles for the USA) are saved because they are representative of a country, as a synecdoche and national symbol.
Therefore, given precedent, I can safely say that the Feddies, being human, will most likely elect to save the species that is threatened if they are cute enough. This explains Janeway's attitude to the Ocampa, because they are cute helpless little adorable baby-people.

If you let civilizations develop without interference chances are high that many will become aggressive and hungry for war and conquest. If you think about it from the smallest environment say some microbes on a rotten apple, to bigger enthronements like a forest, or even bigger like an ocean everywhere living creatures fight for survival and often the most aggressive come out on top. Same is true for the cosmos. Klingons, Romulons and Cardassians exist and they are aggressive. One can only guess how many dozens of civilizations fell prey to these three.
Imho the prime directive should be the opposite. Wherever the Federation finds a civilization that isn't capable of leaving their solar system they should be contacted and their civilization should be brought on the right path. Perhaps in the form of deities for middle age civilizations or in the form of advisors for more developed ones. Ofc also technological help but with the right pace.
This would help civilizations to develop faster and better and would increase their chance of survival. Maybe help them not doing the same mistakes as many other civilizations. This would also bind them in the long term to the Federation because the Federation was there all the way. Not just "Hey you discovered the warp drive, wanna join our club?". The thing about it is that while it has sci-fi elements, they're all in the background and don't affect anything. All the questions the franchise asks, and all its themes are about the "magic" (aka the Force and all that) and not the technology.
