It depends on my playstyle, forexample if you want to run a PH Lovecraft game you really have to have a base amount of mods to make it comfy.
On the other hand you can build games around animals, adding dinosaurs and using the DNA mod that allows you to create your own.
You could also go with the Love of the Rings mods or the medhival ones, it really depends on what you want to do. For me personally I love doing all of them, but I mostly just run with the basic mods, going for a vanilla like playstyle with added stuff that doesnt go far to out.
