1) Scenes are spaced out fairly evenly.
1a) If it's a VN then there needs to be something sexy and arousing early on and there needs to be more than one or two sex scenes at the end. Grisaia does this well by having some nudity and naughty talk in the common route. Kamidori does this poorly by having the first 5+ hours almost completely devoid of anything sexy, but once you get your first scene you just end up being bombarded with H scenes until the end of the game.
1b) Having too many sex scenes all the time becomes dull and unless every scene is spectacular. Otherwise scenes will just be skipped over. Big Bang Age does this and I just can't care about any of the scenes past 2-3 hours.
2) Visually appealing.
2a) Characters need to have good proportions and not have retarded looking faces. Breasts need to be something realistically drawn: inflatable balloons like >>62525 just look dumb, but https://exhentai.org/s/081ebf198f/878162-142 looks good.
2b) Character needs to be visually distinct, preferably without resorting to each girl having a different anime hair color. Kara No Shoujo does this perfectly.
3) Scenes need to be varied.
3a) Even a vanilla game needs more than just missionary/doggy/cowgirl positions. Kama Sutra exists, no reason for things to be dull.
3b) Scenes between girls need to be varied too. If every girl's scenes go something like this: "Noo don't lick me there it's dirty!", "No, it doesn't hurt, you can start moving", "Ahh this feels so good, I love you MC-kun!", "I'm cumming!", then it means your H scenes are awful. Every moege is guilty of this. Fuck moege. Rance games are the best for this: tender loving, tender raping, sex in weird places or situations, etc, etc.
4) The setting is important.
