For me the most important is the attachment to characters.

I've bitched about it before and ill bitch about it again, games need to change dialogue (and if possible, expressions) from old sex scenes as the relationship with the mc changes. It really shouldn't be that hard to do and makes the game feel like it has much more content than it really does.
There's also an awful habit of 1-scene-per-position in these games. Just changing the context of simple penis-in-vagina sex can make a scene more erotic. For example, finally reaching sex with a character doesn't really feel like you accomplished much if they'll only have sex in just in the same place and at the same time ever time sex occurs. Its much more erotic if the mc moves (as the relationship progresses) from having sex just in bed to sex around the house to your partner forcing sex in a public restroom or something.

1) Scenes are spaced out fairly evenly.
1a) If it's a VN then there needs to be something sexy and arousing early on and there needs to be more than one or two sex scenes at the end. Grisaia does this well by having some nudity and naughty talk in the common route. Kamidori does this poorly by having the first 5+ hours almost completely devoid of anything sexy, but once you get your first scene you just end up being bombarded with H scenes until the end of the game.
1b) Having too many sex scenes all the time becomes dull and unless every scene is spectacular. Otherwise scenes will just be skipped over. Big Bang Age does this and I just can't care about any of the scenes past 2-3 hours.
2) Visually appealing.

2b) Character needs to be visually distinct, preferably without resorting to each girl having a different anime hair color. Kara No Shoujo does this perfectly.
3) Scenes need to be varied.

3b) Scenes between girls need to be varied too.
