I dunno. I do like the way Shirow does those little boot legged gremlin bodies and it does actually give his versions a more unique look. So many others do their SD that way, such as pic related. That is a very nice Moka though.

oddly enough, going full chibi for a whole comic seems to work a little less well. Phantom of the Audience, while nice, makes the smol stuff look a bit less impactful since it's normal

Yeah; it's best used for effect sparingly; I noticed that Shirow in a lot of his comics tends to save it for either certain specific comedic moments or for punchlines at the end of chapters.

Well, that's a rather hard thing to do. The initial idea that comes to mind is to go animation, a'la Rodger Rabbit, have them shrink down, etc and go 2D for the moment. However that would clash a bit and unless done properly, it would look weird, and more importantly, it would be incredibly expensive to pull off. So unlikely to actually be done.
The second idea is to have an SD puppet costume which the actress would 'wear', putting her head into it while it's being manipulated, as you'd just use it for simple SD expressions and movements. It's a cheap option, and it could work but you lack the SD proportions of the face and the movement would be very limited due to how you'd have to work the prop/costume. That said it would have the actual actresses head.
The third option is to have a midget play the SD form, with either a mask or face covering helmet with a fake face on it and a costume to make the actor/tress look less like a dwarf and more SD. The pros of this is a wider range of movement and ability to do stunts, action stuff, etc while SD and you can have the face be the right SD proportions; the downside is that you would likely be stuck to single expressions and it would look weird if you did animatronics (plus more expensive).
Most likely a balance of the three would bring the best results, with the second two most likely (with the face being hidden with the small actor/actress if you want action or movement).

That'd be a bit extreme; I'd suggest going for something less literal; simply have her hair puff out, film her with a bit of a fisheye effect and from a high angle, while she goes all mad atcha; that way you get the gremlin effect without it being too surreal.
