The artist will also need to draw costumes for the various characters involved in the game. This is easier if the majority of the characters are women. There's more reference material available, and you can use common articles of clothing (such as swimsuits and skirts) without worrying about weird crotch bulges.

The writer needs to establish personalities for the NPCs. If they're women then he can rely on common tropes (maiden, tomboy, femme fatale, etc). If they're futas then they'll require more bespoke work, and he'll need to resolve a bunch of personal backstory baggage for each one (ostracism, gender confusion, attitude towards child-bearing, etc). A futa PC *should* need to deal with similar issues … but the writer can just opt for a mostly-silent murderhobo PC with a vague backstory (and/or amnesia) and thus skip the tricky work.

The writer will also need to establish the setting (i.e. world-building). The player character benefits from generous suspension of disbelief. The PC is going to slay thousands of trash mobs before reaching lvl 50 and facing the final boss. They probably have a mysterious bloodline, dark secrets, dead parents, inexplicably spiky hair, etc. They can recover from life-threatening injuries by resting at an inn. They can fit 99 chainmail hauberks in their backpack. It's not too much of a stretch to slap an extraneous penis onto a feminine PC, because the PC is allowed to be an *exceptional* character. NPCs don't get the same dispensation. If half of your roster is hermaphroditic then the audience is going to expect some kind of explanation - which must then be woven into the history and society of your fictional world. It's easier to write a generic world filled with males and females (plus one weirdo), instead of explaining how a culture incorporates additional genders into its politics and religion and families (c.f. "The Player of Games" by Iain M. Banks).

Assuming that we have a collection of physically attractive futas in close proximity to each other (during VN dialog scenes or RPG team battles or whatever), the writer must explain *why* these character are solely interested in pursuing the femPC. Why don't the futaNPCs just fuck each other whenever they get bored or horny? If the NPCs are women then the answer is simple: they're hetero, they want the D, and the futaPC is packing twelve inches of meat.

Fans are reasonably accustomed to a male-dominated harem; this is often treated as the "true" or "best" ending of a dating sim game. A futa PC with a harem of girls fits into this model reasonably well. A female-dominated harem is going to be more controversial. If the "true" epilogue of your game involves the femPC getting multi-spitroasted by seven dicks … then some fans will categorically reject the whole thing as ntr/cuck.

If you're planning a game around futa-on-female sex scenes, then there are market factors and development challenges which will push you towards futaPC+femNPCs. A game based on femPC+futaNPCs will likely require more work and sell fewer copies. Therefore it's rare.
