There was no single CDC-recommended PEEP setting for all COVID-19 ventilated patients. Early in the pandemic, hospitals usually followed ARDS-style protocols, with many starting around PEEP 5 cm H2O and then titrating upward based on oxygenation, sometimes into the 10–15+ cm H2O range depending on the patient and local protocol.
The recommendations were generally written by critical care / respiratory medicine groups, hospital teams, and ARDS protocol authors, not the CDC as the main source for ventilator settings. The CDC’s role was mainly in infection control and surveillance guidance, not in prescribing a universal PEEP level for COVID ventilation.
A practical example: one COVID-19 hospital protocol from March 2020 started with PEEP 5 as a minimum and used a table that could move PEEP up to 22 cm H2O depending on FiO2 and oxygenation goals.

High PEEP can contribute to barotrauma by raising alveolar pressure and, more importantly, transalveolar pressure enough to overdistend fragile alveoli until they rupture. The key idea is that PEEP itself is not automatically harmful; it can actually be lung-protective by preventing repeated collapse and reopening of alveoli, which causes atelectrauma. The risk rises when the pressure is too high for the patient’s lung condition, especially if the lung is already stiff, heterogeneous, or severely injured.
So the problem is usually not “high PEEP always causes barotrauma,” but rather excess pressure in a vulnerable lung. That is why ventilator settings are balanced against plateau pressure, driving pressure, oxygenation, and the patient’s overall lung mechanics.
