I was playing with the files from Ultima: The first age of darkness (1981 Apple] [+) and I found a file called "PIC.ULTIMATUM" which is a PNG (format that exists since 1996) and you can check it for yourself same with AppleCommander.
My question is: How is this possible?

Obviously it was added by whoever created the .dsk file you're using.
An actual raw Apple Hires image wouldn't have the green/purple color bars.

The Apple didn't have color RAM or the like, and the program and hardware only know monochrome bitmaps. The fake color is added by emulators to emulate the NTSC chroma phase effect that the Apple][ used to produce color.
The dsk format only showed up about ?1996 (.do and .po was in use before then). That PNG was probably some embedded thumbnail used by a DSK Browser program.
