America has a number of issues since the Orthodox Church has about three or four renditions here (there's the Greek church, the old Russian church, the Soviet-era church, and the "new" American church- they're all officially recognized as Orthodox but none of them are recognized as the one Orthodox church in America). At the local level we're all Orthodox and it doesn't matter and we freely use each others churches, but at the national level (and political level) we're separate entities and it's a jumbled mess in America. If you have to have a top-down structure, at the international level at least, technically my church would respond to Metropolitan Isaiah (Chronopoulos) of Denver since he's a sort of "regional director" and he'd respond to Archbishop Demetrios of America, who is the closest thing to a "internationally recognized patriarch" in America.
