Not at all. It can easily be boiled down to a short intro explaining how Toga has kidnapped Midoriya and is now in the school acting as him with the goal to train the girls for lewd stuff or win them over. Thats all of the setup needed.

During gameplay just add some hints here and there like a girl noticing how Midoriya is acting strange or asking questions. Occasionally Toga could be shown as overlay commenting on others or the situation with her own character as inner voice.

Thats pretty much the same setup the four elements trainer has. It's even easier to do because MHA has already the characters with the proper skills and motive to do it. Four elements trainer had to insert some strange plot for it to work which isn't necessary here. If Mity is able to do alone a complex version of it a simplified one can't be really to complex.
