>How does a country that does not have a royal family go about establishing a monarchy?
That depends on the country and the situation the country is in. If the established country is stable and prosperous, it would be hard to establish a legitimate and recognized monarchy short of a hostale takeover from an outside empire or very VERY popular military/executive leader.
If the established country is on the brink of collapse, a peaceful takeover can be achieved by convincing the people to force their legislature to give up their sovereignty to the monarch or by making the established government irrelevant.
In short, establishing your own dynasty in a nation where royalty is foregin or verboten is no easy task. There is no easy answer to this question.
>And how would people follow a man that is proclaimed king in this secular liberal society we live in?
They wouldn't, secular liberalism will die out or liberals will run away to a nation that conforms with their political animalism.
