The biggest appeal is being able to do things that are currently impossible in real life, like fucking (or being fucked as) a robot, alien, monster, or what have you. I think what I like most about it, though, is being able to do things I can't or would be hesitant to do in reality (kinks that are messy, dangerous, etcetera), and how in a way it's "hyper-real." By which I mean there can be as much or as little focus on the detailsthat otherwise might be lost in the heat of the moment or overwhelmed by the blunt feelings and sensations that come with interaction in meatspace.
If I had a girlfriend or spouse, I probably would stop unless they were into it too. Or, if they weren't into it but wanted to get into it, I'd introduce them to it.

I basically don't deal with it at all. That's how.
My RL life is my RL life, and my RP life is my RP life. Neither seeks to compensate for the other. It's just something I do, nothing more. Some people play sports, some people are in a hobby club, some people work on a project - I fuck imaginary people with the power of my keyboard.
That said, my RL life is fairly bland. I can go to work as any human being no problem, but social situations otherwise completely drain me, FAST. I'm crowd-deaf and can't make out what people say at the pub if it's the least bit noisy. After work, I go home, fix myself some dinner, and then play video game until bed time. I keep F-List open in a separate window and pay an eye to the ongoing chatting now and then. If somebody interesting shows up, I latch on. Otherwise, I just keep gaming.

My wife knows, and doesn't really care. We met while RP'ing non-lewds about 20 years ago, on AOL. We have an active RP between ourselves, though. Cast of thousands, no joke, and we voice-act everything. Which is what we're doing as I'm typing this. As expected, we have a lot of couples - though we switch off who has which role in the relationship. We even have a few gay couples. It helps that my wife preferred playing as men when she was online, and I was the first to RP with her playing a female.
