I never really understood why age regression in media seem to always involve cloth diapers. Kids nowadays probably can't even recognize a cloth diaper with pins.

Probably the same reason that the floppy disk has become the universal symbol for "save".
Somewhere along the line the cloth diaper became the symbol for diapers and infancy in general. That symbolism stuck even after the technology became largely obsolete.
