It's a type of spica cast shown – following a femur or pelvic fracture. These are significant and potentially fatal fractures: the pelvis is very complex and contains a large number of vital blood vessels and nerves, which, if disturbed, can kill or paralyse the patient. The bar is there to prevent adduction of the leg and fix the position of the neck of the femur until it heals – i.e. to fix the "known-good" portion of the femur in the right location and prevent it moving. They're typically applied under general anaesthetic, and often have to be on for 6-12 weeks all the way to 6 months. I suspect most of these are fetish performers, but at least one of the pics is real – you're probably jerking off to a girl who was seriously injured with bilateral leg fractures in a car crash.
Adult patients can use a bedpan, but it requires another person to help. Personally, I'd pick the diaper…

But you forgot to mention, hip (spica) casts do not work on adults and therefore all the videos and images are not and can never be real.
For adults they will attempt an internal fixation or a hip replacement but spica casts are not an option past the age of 6 months at all.
If you see anything with children older than 6 months then it was due to a post surgical or s/p femur head fracture and even that is falling out of medical efficacy.

Two random things: (a) speaking of pelvic fractures, have you ever had instagrammable kids asking 'why does grandmother have #NOF written on her notes' [here # is medical shorthand for fracture; NOF for neck of femur]; and (b) I WISH someone in medical procurement would actually try diapers at some point. The ABDL ones are SO much better. My hospital has shitty thin and leaky cheap crap that I am sure is more expensive in the long run. The patients could at least be dry with a bit better.

As for the diapers it makes sense. As a hospital you want to minimize adverse reactions and diaper rash is an adverse reaction, so it would be better to change 20 shitty diapers than 2 fully used good ones since risk of skin injuries is minimized. They also would prefer to put people on urinary caths in order to collect more from insurance as that counts as a medical procedure while changing a diaper does not. They use the lack of skin injury to justify it, but now the increase in nosicomial infection in the form of UTIs is causing insurance not to pay out so they are starting to swap back to diapers/letting patients piss and shit the sheets and just changing those.
As for why the ones they use are shit, the diapers bought are purchased for pennies a diaper in bulk compared to the amount needed for better diapers, so it is just better for the hospital to just use cheap shitty diapers than good quality ones.

Nope. Almost all medical abbreviations that arent just the first letters of what they are referring to (ie GERD, TKA, etc) or the latin, are just made up by someone and got popular because it made some semblance of sense. Personally I've only ever used "fx" to refer to fracture.
