Well, it's pretty much that the "clans" a.k.a the organizations put their teams in there and then they ladder higher to fight for the spots, and the worst ones just fall out.
Sometimes you just watch matches where they steam roll the other team with ease and 0 rounds lost.

I would if I had the game and the motivation to do so, but those games are pretty easy considering that I've already played Dead Space 1&2, Singularity, both killing floors, both nazi zombie armies, all STALKERs, all Wolfensteins, and both Metros.
