Some ideas I might suggest would be adding pregnancy to characters who lack it. Good, current examples being Rubi, Lottie, Katherine and corrupted Whitney. From most to least work. Arian could use it too, but she's a follower so approach cautiously.

If you were down to grind, there's also defutaing characters who've been ignored like Minerva.

There are followers with a lot of room for expansion like Vala, Ceraph and the Milk Slave.

Or you could always dig up some obscure character, like Helia's sister you meet in the tower, then only ever see again in that birthday event, and make her more. She's still kicking around the city, after all.

These are just some ideas off the top of my head, though.

Ultimately, try not to overthink it. Pick something that feels right, and roll with it.

Or just play Morrowind. It handles level-scaling just fine, with a good mix of hand-placed static loot/encounters and leveled lists that select from a pool of non-scaled choices (so you might encounter a dremora or a dremora lord with the chance of each being based on your level, but the dremora will always be the same strength and not scaled to you). There's some level-scaling to keep things interesting and difficult, but there will also be harder and easier areas and places with high and low level loot. It's only the newer games that fucked everything up.

The optimistic route could be nice. We've had our fill of somber and serious characters for a bit.

Same goes for her wanting to fight demons but not really being sure where to even begin making any difference. Maybe she wears some imp bone stuff to show she's not 100% incapable.

Though I would avoid the kidnapped family angle unless you want to go as far as including some sort of quest for trying to find them. Though keeping in mind that even if it were as simple as taking her to the factory to check if they're among the freed captives still slumming it, you'd still have to write out various stuff for saving and interacting with them as well. I advise starting small, and so maybe she escaped a waylayed group of merchants she has no personal connection to. That might be a good place to start. Hired as a guard while the guy/girl is going through the desert, get captured by sand witches, sand witches does some experiments on her resulting in her current state, she escapes and now just tries to fight the corruption in the desert.

I honestly don't think it needs to be any more complicated than that. And as she has no real connection to the person there's no need to make a rescue quest for them.

I'd personally go with ice magic being heavily buff-debuff based, personally.

Most could do middling damage, but tank their speed. Though it could also reduce accumulated lust, and apply a resistance to it. Possibly a minor damage resist as well, due to numbness.

But as you said, consecutive attacks stack the damage as the slowly freeze. Freezing could also cause fire to do extra damage, at the cost of removing all frost debuffs when you do it. If you've even gotten so cold you went numb, then got in some warm water, you'd know how much it sucks at first.

Additional spells could be a version of the main spell you cast on yourself to remove and resist lust, at the cost of a lesser version of the normal penalties of being hit with ice.

And an ice spike ability that does higher damage to an enemy, but with fewer penalties. However, it may bleed when it thaws in x turns.

Like when I made Terrestrial Fire, I currently plan to have it come through a quest, though a longer one than that, with the magic granted in stages. Two endings that change the magic: One is stronger but with a permanent debuff, but also has a special once-per-day effect
