The first one is the biggest, since a lot of old online games only sold "a spawn" or just the data needed to connect and play, not run or host. Literally nothing else matters here because the rest are "failure to follow rules", which are extensions of the subjective and shady nature of their authority. They can enforce anything and essentially have no checks or balances in regards to fines. This is how they enforced the system, they act like tyrants and are gatekeepers. If they decide that a certain type of game should be AO, they can.
Of note is that they don't actually care about what passes if it's under their political radar, they passed A Hat In Time to "rated Everyone" despite the fact that there are several mature sections related to suicide and a few questionable dialouges, one specifically in the forest level where you swing off of nooses to travel around and they talk to you about how they want to hang people.
