Usually there are a small number of interesting/fun options and a whole bunch of boring/distasteful/impractical options.
I've been thinking about designing one which allows you to choose what rules to follow from a large list, but uses dice rolls as modifiers to those. This would have the advantage of letting everyone choose the ones they like and avoid the ones they hate.
It would work a bit like a tabletop RPG. You'd have a certain number of points to spend on creating your "character". Some rules would cost points but make things easier for you (eg. letting you use the potty from time to time). Other rules would give you points but make things harder (eg. restricting your clothing choices). Dice rolls would be used for things like deciding whether you can hold it long enough to get to the potty. You could also earn points as rewards for doing well (eg. keeping your pull-ups dry), and those points could then be spent on other things.

How about …
Find BMI and weight needed to lose to reach normal dice roll is percentage of lb difference between normal weight and current.
Dice roll is also equal to diapers to wear without breaks
lbs lost is equal to number of days you mut wear within (if roll > than pounds lost, must continue until roll count is met)
Eg rolled 10 bmi says should lose 50lbs to normal. No diapers until 5lbs are lost, once completed you must wear 10 diapers over 5 days.

Been thinking about a similar thing, but haven't put any significant thought into it.
From the ones i see the big challenge is balancing the experience since most of the options usually go from 'just casually wet your diaper' to basically just destroying your dignity on the worst case scenario which is not exactly fun.
Also noticed that on some of these very low rolls on diapers can be as bad or worse than the high ones, like getting thin diapers and requiring to mess a lot.
Some are good, but I'm turned off by the idea of pooping myself, and rolling again takes away the fun after a couple of times.
Maybe we could come up with an rpg/dnd like game? One which you could do in sessions when you have the time and privacy.

So I got bored and was trying to relearn python. So I took this as a challenge to figure out when this dare is likely to end, if at all.
I made a few assumptions:
Normal diapers last 6 hours, double diapers last 8, triple last 10, quiet diapers last 4 and diapers you must mess last 4 (to since once you go most people change).
I also set a maximum number of changes at 1,000 and did 10,000 trials. So here's some stats for those interested.
Just under half the time (49.5%) the dare ends before the first change. It would be 50/50 but the chance of being double diapered or triple diapered and rolling a -6 bumps it down some.
After that about 34% attempts within the first 9 diaper changes. Another about 10% end within the first 100. Then it declines with about 1.6% continuing to the maximum 1000 changes.
In these systems, the dare last less than a day 57% of the time. Less than 2 days (but more than 1) 17% of the time. 3 days to a week 14% of the time. More than a month 4.9% of the time.
Anyway, as expected by the nature of the times, it's heavily front loaded as "most likely" it will be short. But once it gets past that hump it has a good chance of continuing for quite a while.

People might find these interesting. The first is a more sophisticated potty training game with some interesting mechanics. The second one is pretty simple but I found it really arousing for some reason.

I also have some ideas for a phone app which handles all the dice rolling and basically acts as a virtual mommy. You could press buttons to ask for permission to use the potty or report an accident, and it would keep track of your progress and hand out punishments when necessary.
