It's mostly self-explanatory from what I remember. You can fiddle with the settings in the preferences tab (signified by the grey gears) and in there you can choose your options.

You'll need both to hook and translate the game. Start up ITH first and then TA. Then start up the visual novel. In ITH look for the game's exe and accept it. The in-game text (Japanese) should show up after clicking on the game itself to progress the text, and if it doesn't then open the little bar on ITH and scroll down until the text appears legible. In TA you'll want to choose Google for the translator and keep connected to the internet to work.

I'll admit Miyako's route was alright but it wasn't pure - they had sex IMMEDIATELY after the confession scene. The one with the sword girl was pure, but every other one was quite lackluster, and the German girl's one was downright awful and as far from vanilla as possible. That's not even getting into the sideplots of their respective routes which are mostly shit too. I liked Cap but his route was awful, too. It's like he learned nothing in the end. I also wasn't a fan of how the protagonist was basically omniscient, but whatever.

Pure love is more like Katawa Shoujo. CLANNAD is pure enough but I haven't played through all of it yet so I cannot make a definitive answer just yet. So far it seems very pure. But take that with a grain of salt until I return (probably when this thread has reached the end of its lifetime) having had finished the game in its entirety.

I'd also like to suggest Yume Miru Kusuri, a fairly commonly suggested VN, but with good reason - it's fucking amazing. Only 3 routes but what it lacks in variety it makes up for in quality. Has tons of premarital sex but whatever. You may notice this VN does a few things that no other VN has ever done, and that's partly why it's so great. Give it a try, but don't be too bummed out when you realize the sister doesn't have a route.

Look, it's not my cup of tea either but the game pissed me off trying to find a working copy and most torrents I found were dead or strangled thick so you got half a bit every fifteen minutes download. No. The closest you get is in V3 where there's a love hotel, which you can't enter but at a specific point winning a particular key item and deciding to use it after lights out will have you fantasize about you and a random other character who isn't dead at the hotel, at least the lead-up. It cuts to black ellipses though and doesn't show anything. Look at Urushihara; he doesn't have a lot of stylistic range (you can always tell it's him) but within his style he can do loli, catgirls, guys, big dudes, bishounen, monsters, Christmas Cakes, etc. The man is absolute inarguable genius–and yet he will never not make his character's have almost metallic-shiny hair and look like quintessential 80's anime. Midway through chapter three, but it's alright so far. Not really into the sex scenes, some are good, but it's got a few genuine laughs and lot of funny porn-corny dialogue/scenarios. For instance, I just finished a scene where a ghost possesses the body of her little sister (your friend) and tries to drag her to hell, because if she won't allow herself to be happy - then the ghost might as well be together with her sister in hell. It's almost comical, but the game treats it seriously.

Started reading Memento of Spring. For an ELVN, it's way better than expected. Most of it is actually pretty fun to read, the writing style is very similar to a well-done fan translation of nip VNs, to the point it's easy to imagine the "original" lines in Japanese as you read along, and devoid of any shitty memes. The art is a bit on the low budget end, but still decent, and after paying less than $1 for it, I kind of feel bad for the author. A $5 or $10 pricetag would be perfectly fair, and that's coming from someone who does not pay for digital content often.
