Tim Cain has mentioned in some interview somewhere he'd actually like to release the source code for this and Arcanum, but the contract prohibits him from helping bypass the DRM it has been sold without for years and nobody at the companies involved could be assed to help him.

May have gone over a the heads of people not familiar with 3.5. Except for the Warlock (which is a bit more complex but not a huge undertaking) all those classes have at most one unique abilities (and the ones they do have aren't that complex, for example Scout is just Rogue with a different style of sneak attack and some ranger abilities) and would actually be quite easy to implement, giving an appearance of new content with the laziness one would expect of Beamdog.
Red Hand of Doom is actually what I'd have picked for an expansion pack. The module is well received, needs minimal new content for a video game and most of the encounters can be run as is. The game world is fairly dynamic and non-linear.

Good mod tools (whether fully official, or merely the result of better documentation/understanding of the game) really would be a godsend. Co8 has gone through a heroic amount of effort, but the mountain of barely documented spaghetti code making up their ToEEFE-X toolset is in dire need of replacement or tremendous improvement.
I'd really like it if there were lots of "total conversion" mods for ToEE, since its design (prerendered backgrounds, 3D objects, turn-based combat, actual party control, near-perfect computerization of D&D 3.5e ruleset, pie menus, etc.) so perfectly fits my tastes.

The editors used internally by devs are rarely up to snuff with tools intended for public consumption, especially since documentation usually takes the form of "walk across the hall and ask the guy who wrote it for you". They're also invariably WAY buggier.
Polishing it up beyond bugfixes, or at least sitting down and writing a decent manual for it, would make a tremendous difference.

By modding something such as ToEE instead of making your own game, you can do stuff using actual D&D, instead of a bland-i-fied D20 knockoff like Incursion or KotC. Further, by using ToEE, you get access to what is by far the most popular version with the most material, 3.5e. Also, of course, ToEE already has lots of quality assets suitable to reuse and modification.

The closest anybody ever came to an official GURPS game was Fallout. The deal fell apart due to SJG thinking BIS was too edgy with gore/sex/politics/etc., so the underlying system was retooled into SPECIAL during development.

It was most likely a combination of the dark edgy humor in the Vaultboy pics and also the other edgy stuff. I'm surprised Steve Jackson games didn't stamp their foot down on childkilling considering Germany censored the game because of that. Fallout losing GURPS was a blessing in disguise as the SPECIAL system matched the game far better and was just the result of the developers taking GURPS and making it fit their setting better.
