There's always SS13 (and with it, the new ayy lmao servers from the leaked code that has more advanced medicine (more surgery, toxic shock from busted organs, oxy loss from damaged hear/lungs ect.)
It's a shame hatestation isn't around anymore, it has some interesting but also very buggy dismemberment code. Most of the time you just chuck people into cryo unless they have appendicitis or the server has the bone breakage system in place.
That being said, you'll have to deal with your medbay being bombed on the reg or some other stupid shit happening unless the server is low-pop/dead-pop but then you have no one to treat, so if you want a focused single player experience it's not the ideal game.
