How does this achievement work exactly, if a person logs off after losing, you get the achievement? Or do they have to leave right after you kill them?

You have to dominate the enemy player, i.e. kill him four times without being killed by him. After that, if he leaves the server, you get this achievement.

This, the domination system is basically a salt generation mechanic. It shares a place with random crits as being one of the earliest signs that the TF2 team didn't know what they were doing.

They knew exactly what they were doing with random crits. Without random crits, low-end players are blocked from ever being able to compete with high-end players (see pretty much every arena shooter for a good example of this in practice). Getting stomped? Might as well just leave the server. With random crits, you may get completely outplayed, only to gib the other player with a lucky crit grenade/rocket.

That right there, the ability for a shit player to kill a significantly better player based on pure luck, was a huge factor in TF2 becoming a juggernaut.
