I'm not sure. The main example I can think of is a DMM.com game on Nutaku called Brave Girls Raven. The (((localization))) team did real full-blown decensoring of all mosaics for all the girls H-scenes. However, they also slapped giant tits on all the lolis that look really weird and out of place. They also got called out on it and kvetched heavily. Then ultimately did jack shit to fix anything, even though they promised they would. Then stopped translating or (((localizing))) new content entirely. And the game has since died. It was actually kind of fun until I realized it was dead and the lolis were censored with giant tits.
In this game, the loli nudity is covered in towels or removed from scenes, but it's the same concept. I'm not sure who did the decensoring for Brave Girls Raven. But it makes me have little faith in companies to actually deliver on their promises for this kind of stuff.

They deleted every single comma from Chinami’s lines. All of ’em. Even the ones on the slow, emotional lines. Even the ones that are grammatically required for direct address, to avoid ambiguity. Every last one.
The translation constantly abuses the English language: transitive verbs have their object stolen by incorrect prepositions (“I relish in my memories…”); idioms are used incorrectly (“spinning on our heels” appears where they meant to say “spinning our wheels”); bizarre phrases are made up which sound kind of like idioms, but which actually aren’t (“stroke of sharpness”, by analogy to “stroke of genius”, I assume, except nobody in the history of the world has ever said the former until HoshiMemo’s localization dreamed it up).
Above all else, it’s full of lines which are straight-up poorly written. Nearly every time the script tries for a dramatic narrative line (and believe me, it tries all the time), it fumbles for a verb, usually coming up with “to be”; latches onto some bizarre adjectives and arranges them poorly; and then slathers the mess of a sentence in adverb sauce. The end result invariably sounds like something a middle schooler might write

So that's the history of Sekai and Fakku's partnership – Fakku never approached Sekai as the choice localizer for Maitetsu, but rather Sekai approached Fakku as the distributor for the 18+ edition of Maitetsu, who they sold the exclusive rights to.

Besides the only reason companies censor games isn't because they want the game to appeal to a wider audience or that it won't be allowed on consoles. It's to either justify working on the product longer (like with 4kids) or the content personally offends the publisher bringing the product over. People don't pay attention to ESRB ratings anyway and only determine if games are for children based on the box art. Meanwhile anything is allowed as long as it's not adult material so censorship is never "needed".
