Good 'old fashioned "Get touched with this green stuff and it's gonna mutate and burn like hell".
Rather annoyingly, I don't see many mechanics for random mutations or mutagenic exposure in Tabletop Games. Annoyingly, even TMNT&OS lacks it. And the other half of the time, they have all the undesirable outcomes as things you can roll.
I don't think I've seen a system where you're guaranteed to get something beneficial and then has you roll for a side effect.
What's your opinion on mutations as a gameplay element?

Mutations have kind of been fixed in the domain of post-apocalyptic games. Most fantasy games shy away from them entirely, since anything that isn't a buff is suboptimal and will be avoided at all costs. That said, I agree with you and I want more of these things in more games.

Gammaworld had pretty good random mutations.
The novels were interesting, the characters would wake up with wings or antennae and they'd just deal with it and move on.

d20 Modern has exactly what you're looking for. There's variants in both d20 Apocalypse and d20 Future. Full rules for the future version are in the SRD.

Pathfinder also has some stuff with mutations due to expanding Expedition to Barrier Peaks into an entire country giving plenty of places for comic book radiation to exist. Just search d20pfsrd for mutant and you'll find all sorts of stuff. The mutant creature template is most in line with what you're looking for, but there's player options that give mostly straight positives.

There's actually a PFS module that gives characters an arbitrary cosmetic mutation of their player's choice. Can't remember the name though.
There's also this bit from The Reed Richards Guide to Everything (a book for Marvel Super Heroes Adventure Game - SAGA Rules). I recommend tracking down a download for the book even if you have no interest in running the system purely for the often hilarious in-universe articles that serve as a framing device.

It should be considered as a *setting* element first. For example, in 40k it's obviously a thing. In most settings it's retarded.
