You rip the tattoo texture too and lay it over your body texture in another photoshop layer. the maximum texture size in sl is already 1024sq so chances are they'll line up right away. if not its probably 512 and then you just need to double its size. then save as .png upload texture, add new body textures with shiny new tattoos, ???, profit.

Like others have said, it's the render complexity number LL calculates for your avatar based on the amount of tris, texture data, transparencies, etcetera. The higher the number, the laggier your avatar is.
As a general rule of thumb, aim low. 50,000 is a good number to aim for. You might not achieve it, but if you aim for that you should at least keep your avatar in a reasonable range. Most of my avatars are under 80,000. As long as you're under 100,000 most people should be able to render you.
If content creators were optimizing their work it would be easy to keep even the most detailed and impressive looking avatars well under 50,000, but they're not, and SL doesn't really provide any tools to help you reduce your draw weight, so just do what you can.
*Detach invisible attachments.
*Remove attachments that are hidden inside other attachments.
*Replace sculpt and prim attachments with low poly mesh alternatives.
*Change your avatar's blended alpha textures to masked.
*Go into wireframe mode, get rid of everything that looks solid.
