How to convert in Poland?
I love this religion and I want to be a member.
This religion reflects all my personal values.
I always hated christianity, even when I was kid. This religion is scam.

Thank you brother, but there are virtually no Muslims in Poland. There is Imam of Poland named Tomasz Miśkiewicz (he is 'Mufti').
If I went to him and asked him for conversion and teachings, could he accept me? Is it even possible?

Yes, he should help you out as it is his duty as the Imam. But you don't need an Imam to convert to Islam, you can do it on your own if necessary. It is good to get into contact with other Muslims, even if they are few.

I have one more question. I am still catholic, should I first officially apostate? In polish Church this is not easy process because polish priests are douchebags.
Or can I convert to islam without official apostate?
