The reason for it is that it could possibly destroy the mans' ability to reproduce. Why should his wife be childless because her husband hit somebody?

Yes but that doesn't explain why the scenario described is so specific. Why not just have a general "Don't grab a mans balls" prohibition rather than "If you're fighting with another guy and a woman grabs your nuts cut her hand off" rule?

This. It was a largely lawless time time (hence God stepping in and laying down the Law) and I imagine fights between men were extremely common. Wives rushing in to help their husbands with a timely nut squash must have been fairly common as well.
