Personally, I would think of it as similar to the issue of castration as settled at the Council of Nicaea. If you're deliberately dismembering yourself, then it's an affront to God and gravely sinful, but if you lose a limb in an accident or done against your will by someone else, then it's no fault of your own and getting a replacement could potentially be acceptable

Anything that would make you immortal though would certainly not be okay.
