They're not out to kill her at first; Claudia believes that a God born from "pain and suffering" (such as they inflict on Heather throughout the story) would be "more sympathetic", so it stands to reason that Claudia's toadies would want to maximize Heather's pain and suffering prior to the God's birth.
Also, if you've ever seen the occasional special death cutscene(s) where Valtiel drags Heather's body away, that seems to imply that they don't need her alive to complete the process (or that they can return her to just enough of a state of "living" to complete the God's gestation).

Scariest and greatest? Subjectively, not for me but I did enjoy it all the same. It was nice to finally get the payoff on the Walter Sullivan lore from SH2, in the form of a twisted, evil Kurt Cobain look-alike. And yes, I took the Shabby Doll and put it into my item chest during my first playthrough. Never. Again.
