It's randomly generated, but the access points for different branches are always on certain floors. The only times you'll get true randomization are when you go to the Abyss or Pandemonium, or when you occasionally get an event portal while you're crawling.
And yeah, they really do remove shit to cater to speedrunners. It's the most backwards shit you'll ever see. They could just make an official speedrunning mode if they *really* want to cater to those fucks, contrary to adding cool shit to the game and then removing it later.
