Vanilla. Anything else, including mods that are considered good gameplay enhancements, will alter your experience and sometimes make you over/underpowered when you shouldn't, and will make you miss out on some of the ingenious gameplay designs, even when it fixes some of the bad gameplay designs.
Stick to vanilla for first playthrough. Afterwards, consider a gameplay rebalance mod.

GMDX is one of the best mods I've ever used, if not the best, in terms of actually improving the game and not just adorning it with extra grafix and features that don't work and stamping a "MOD BY 666XMetADragoNFuckeRX666" on the menu screen or something. It's really, really good.
However you should still play vanilla first so you can judge the changes for yourself.
