First run of a game should be as mod free as possible, some games are better off for fan patches, resolution fixes etc. and then for the second run you can mod in graphics and texture packs, some tweaks here and there.

Depends on the game. A lot of games don't go past cosmetic mods. In general I usually play my games mod free for the first time and later add fan patches that address gameplay issues and bugs. There are tons of games out there I wish had mods but don't or only have cosmetic stuff and there are many games I can no longer play vanilla, like New Vegas, Stalker or The Witcher 3
