You already answered your one question. To get the best times, every little details of a run has to exactly as planned, or else you lose precious seconds that can make or break the run.
Speedrunning also comes with high mental stress due to aforementioned conditions. You can spend two hours on a run, only to ruin it because you accidentally hit some wall in the wrong way or anything else like that. It's like constantly having a loaded gun pushed against the back of your head, and it likewise is only naturally that you eventually flip your shit over seemingly minor issue.

I get the fact that people like to challenge themselves, but everyone should just stop when the frustration overcomes the fun factor. To a non-speed runner, even the most basic speed run that is way above my skill is impressive enough. I remember watching the very first Quake done Quick and even though subsequent runs have shaved off several minutes from the time, imho they are all equally impressive because of the display of skill.
I don't know if the guys who did them experienced more fun than frustration, but that was the time when people only uploaded the run instead of footage of their ugly mugs.

Me and a friend came up with a challenge to learn a speedrun in a week and see who comes out ahead in a live race at its conclusion. We selected Aladdin on the SNES. It's my first speedrun and it's surprisingly fun to practice playing a game and figuring out how it works and exploiting it. There's a world of difference between playing a game normally and analyzing it so you can go fast. Not to mention it's immensely satisfying to get a good run and beat your previous record by more than two minutes.
Like fucking hell I'm going to go full autism and try to grind for the world record though. It's fun as a casual little challenge, but I can see how being overly serious about it can be a poison for the mind.
