I think there's always at least 25 on at a time, usually more in the 30s or 40s.
The main merit I see in it is the quests, Runescape can be played almost entirely as a single player game, so low server population doesn't affect normal gameplay too heavily, and there's a global chat so everyone can chat with each other no matter where they are.
Security issues are over, originally passwords weren't properly encrypted. Since nothing was actually connected to the accounts though, no one should have been seriously impacted outside of /v/scape unless they were dumb enough to use their normal username and password for other sites on a fan server run by anons.
Nowadays passwords are actually encrypted properly so it shouldn't be able to happen again.
The best thing about Runescape is that the quests are like adventure games, they're more about gathering the right items, equipments, and skills to solve puzzles and things. This is the opposite of the boring WoW-like quests that are just "kill ten ostriches," "collect 100 wool" that sort of thing.
Quests include:

>help a man who's been turned into a chicken become human again
>solve a murder mystery
>help romeo and juliet find love (or not)
>save a saudi prince prince by crossdressing
