Only person you mentioned that actually died but not really. The axel who died was a nobody. The Axel that went back to life was the real Axel, AKA Lea, is the one you see in the cover. This is true for all the organization members, and this happens because when your heart leaves your body it leaves the shell, the nobody, and the heart becomes a heartless. When both of them are defeated, both the body and the heart reunite to return the previous nobody to life. But that only happens when the Nobody/Heartless is powerful enough to be more than a common heartless/nobody.
