Here's the rundown OP(faggot): when you leave novac the NPCs are not even "alive" anymore, in fact the game engine doesnt even renders what's outside your FOV to save on resources. After a certain distance it doesn't even calculates objects, NPCs basically go inactive and reset once you go back.

There isn't a lot of attachment I get out of NPCs in games for a reason. My favorite waifus in vidya are at some point player controlled characters with AI partners being a close second, but only because you have some control over AI partners. The exception may be Harvest Moon or Rune Factory where the NPC Waifus are an objective. But as non-objective NPCs that are there I just don't get a close attattchment. I suppose It depends on the game and how involved that character is in relation to the player's avatar whether or not I feel some attachment. Half the time NPCs are there to exploit for gameplay reasons anyway.

Take care of yourself, lead a life you can be proud of. If you do all this to fix yourself and still no woman wants you, then God has plans for you being alone, accept that. I used to dream that AI would reach a point where the NPCs in games believe they were real and you talk to them through a microphone. And, instead of a story, there would just be preconceived factions and dens of monsters, all with their implanted memories of rivalries and prejudices. And as you went through the game, saving and reloading every time you die, the NPCs would eventually catch on that you are invincible because everything always goes in your favor and you seem to always know exactly what is coming. So, as you become more infamous, the NPCs would get together and secretly study you. And eventually they'd figure out that you have some weird ability to stop and erase time, and they'd conspire against you and trick you into saving while locked in some underground dungeon, maybe even after sending you on a quest there. And they'd all celebrate, thinking you are entombed away and defeated. They'd celebrate until the moment you overwrite the save and start a new game and their world is deleted.

So, after about 20 in-game years, you finally see the tomb door open. And there is a squad of geared-up, overlevelled NPCs who want to make a deal. They can paralyze you with a paralyze ring and lock you in the tomb again or you can help them defeat the evil emperor. Since you haven't played for a few irl months and are not familiar with all the changes to the game, you know it will be too tough to fight them right there. So, you agree to help them, and fight your way to the emperor's stronghold, becoming more and more powerful along the way. Then, right as you reach the stronghold, you uninstall all the mods and return the world back to normal. Then the emperor is back to a regular NPC and you kill him in one hit. All the NPCs worship you as a god and resolve to serve you for the rest of their lives as repentance for entombing you and they make you new emperor of the world. Then there is nothing left to do so you get bored and never play again.

Game would already have aging, breeding and succession systems. So no need to install a family mod unless you want to flesh it out beyond that. Now for world design and politics:

When the games starts, world would be made up of about 20 city-states, each ruling over 30-40 small villages. The cities will be mostly coastal, except for one or two mountain cities, supported by mining villages. Each of these cities is ruled by an independent king. Some kings will be absolutist tyrants, some will be beholden to a constitution and council. There will be craft and trade guilds with a centralized ruling peasant family that also hold sway over the king of the city. Some of these guilds will span several cities. The inland will be populated by several feudal manors with rich lords that rule over farming villages. All of these lords will be vassal to two or three emperor lords that are in a shaky peace at the start of the game.

The world will also be populated by dungeons that act as independently sovereign zones. Some will contain mindless beasts, others will contain bandit gangs or intelligent beasts (like goblins) with their own chieftains and succession systems. These dungeon dwellers will war among each other as well as attack cities and villagers, prompting NPCs to devise plans to clear dungeons that may or may not involve hiring you.

Finally, there will be four or five crime syndicates that oversee illegal activity, there will be cults that practice the illegal magics of necromancy and demonology, there will be a handful of religions. All of these will operate as factions with a centralized big boss and lesser bosses stationed in various cities or sometimes dungeons.

World will be scaled similar to Daggerfall, with vast generated wilderness. There will be nothing of note there when you first start the game but, due to the dynamic nature of the AI, it will soon be filled with camps and forts and hermit's cabins and all sorts of other surprises. Witches can curse patches of woods, armies can clear forests. The player will be able to fast travel, shutting off his need to eat and sleep temporarily and fast forwarding time. However the player disappears from the world when this happens so, if NPCs will be accompanying you on a journey, you will want to avoid fast travel as not to clue them in that they are in a video game.
