Both, look at Mass Effect andromeda, and mafia 3,
For making an immersive setting you need to make a good setting, unfortunately unless you're creating a dystopia on purpose putting tranny lesbian niggers everywhere works against you
Once you have a good setting, for a video game you need to program ways to interact with it, so it's not a wagon ride through a spookhouse, this is a huge challenge for diversity hires, women and niggers, the more features you want to add, the more you need to optimize your code, which isn't taught in programming courses apparently, because you can shill for new (((Nvidya and Intel))) hardware instead
Unfortunately now market is large enough for devs to make lowest common denominator trash and people will eat it up, best example being MGSV

Blame Warren Spector if you want to blame anyone, it's just his name for the vague Looking Glass FPS/RPG/adventure house style. For this reason, most elements in these games are often classed as variously "Underworld-like", "System Shock-like", or "Thief-like". Also for that reason, somewhat similar games from rival lineages, like TES (and the similar Gothic/Risen) or Metroid Prime, are usually excluded.

You never listened to the director commentary? It was literally designed to be an immersive sim.
It just drops you into the mission and let's you figure out the rest. You can do anything you want. if you ever played Thief, you' know that you can play it without stealth. You can go on a murder-rampage, you can stealth the game without even knocking out enemies.
You can just ironman it and do whatever works and live with the consequences. Getting caught, doesn't break the game.
