Difficulty settings are one of the worst things you can do to a game. A well designed game will put a lot of thought into getting things like the damage values of the different enemies just right. Difficulty settings basically allow the player to completely screw with the game design and not only that but they are expected to do so before even having played the game. It's basically letting the final say for the game's balance be made by the person least qualified to do so.
The player has no point of reference to know whether easy mode will to too easy or whether hard mode will too hard. Normally the smart thing to do is pick the hardest mode available and then try it on easier ones if you get stuck. Unfortunately a lot of people fall into the trap of thinking "I haven't played this game before so I'll pick easy" and then end up drifting through the whole thing without actually having played it since the aspects of the game which were supposed to challenge the game had been butchered.

I'll just use Metroid: Zero Mission as an example. The game has Easy, Normal and an unlockable Hard difficulty setting. Nothing is stated in the manual or game about their differences and had to look online for actual answers.

>Hard: Energy Tanks only give you 50 units of energy instead of the usual 100. Missile Tanks only give you 2 extra Missiles instead of 5. Super Missile and Power Bomb Tanks only give you an extra 1 round of ammo. Some enemies inflict more than 50% damage than in Easy and Normal.
It's weird more games don't have sort of training/practice level to experience/explain the differences between difficulty without having restart files. Or do something crazy like determining the difficulty of the game by how well you get through the first level, I was going to use Dark Savior as an example but that seems to change the game story (5 different paths) instead of the difficulty by how fast you get through the first level.

Generally games don't do anything fancy enough that you can guess by context. Chess games improve the AI, most action games tweak damage values, etc. But sometimes they do subtle things. One example I like is Dawn of Sorrow's puppet master boss. On hard the puppets take four hits to destroy instead of two. Use a slow weapon and you're screwed. One I do not is the massive leap between the first boss of Castlevania LoD on normal vs. hard. I can't remember why but I think it was the monster's aim being predictive and hitting like a truck. From there it makes a lot less difference.

You can pick the base dificulty, but then the AI Director will fine tune it. If you're doing well, progressing fast and unharmed, it will throw more hordes your way, spawn less medkits and space out the weapon pickups.
On the other hand, if you're doing rather poorly, your team is severily hurt and maybe you lost someone, you'll find more health kits, more bombs and less hordes.
It's pretty well done because it still works within the dificulty you set, eg: Normal dificulty will never reach Hard dificulty no matter how well you play, nor will it reach easy either even if you suck ass at it.
It's very small details here and there sure, but they impact the game in a subtle way to keep you challenged but not outright slap you out of the game.
But I'd like to see something in-reverse: the more you suck at it, the harder the game becomes. Everytime you die or lose something or miss an objective, the game gets even harder and harder to the point you can't come back from it.
But if you're really good at it, the game would end up being boring, so I'd have a "be good and dificulty the same" mindset and just let the game punish everyone of your mistakes in an escalating manner.

The way I'd do something like this is that every time you die, the Devil offers you the opportunity to sell a part of your soul so you can immediately continue where you died. However, every time you do deal with the devil, you become more strongly infused with demonic essence, allowing you to tank more damage and rip through enemies with increasing ease, to the point where nothing even poses a challenge anymore and the game falls apart because you're so blatantly overpowered.
If you deal with the devil over seven times, you will fully become a demon and you can respawn infinitely. Enemies will no longer bother attacking you but instead flee on sight. However, the final boss would be brutally difficult (but possible) to defeat, repeatedly killing you with almost every strike until you either give in or do manage to kill the final boss. You cannot refund your soul, the only other option is to restart the game (the game itself having the length of a NES platformer) and beat the game fair and square without continues.
Journos would have their Journalist Mode, which technically lets them complete the game, but the very idea of selling your soul should make it obvious that that's not the way the game is meant their played. It's no different from creditfeeding in arcade games, but here a storyfag reason is added for why you'd want to 1cc the game, and journos just can't resist storyfaggotry.

This game is a testament to a herd mentality. There's 9's &10's across the board, and everyone is praising what a masterpiece this game is. The male-pattern baldness is a trait on the X chromosone. If you have it, there's no counter since males have a Y chomosone alongside instead of a chance of another X chromosone without the trait. Lord help if you're a woman with both X chromos with the trait.
