Humans, Human interpretation and ==BIAS== are the problems that humans bring in these type of projects.
There is also multiple legit reasons that some information can be wrong.

By design Wikipedia is going to reflect what mainstream sources say, it doesn't matter if said sources are easily proven wrong (because unless it comes from an accepted source, any proof you present is original research and thus unacceptable), so any subject in which mainstream sources are heavily biased or inaccurate will be poorly represented on Wikipedia. The solution to this would be to evaluate sources by some standard other than "it's mainstream", but that would be pretty difficult to implement. Also, anything that results in important, "respectable" sources being declared not reliable would tear the site apart. Just imagine the butthurt if say the Huffington Post was declared not very reliable.

Check some of noam chomsky's recorded lectures, in particular: Necessary Illusions (4 hours long and gives in 4 pieces an introduction in how the elites force discourse through the media + an introduction in the book i mentioned in the beginning). For moving images and sound, check the documentary of bbc: century of the self (also 4 hours). This documentary mentions the rise of thought control in democracies after ww1. Key people to evolve this theory of thought control: Gustave le bon (the first person to write about herd behavior), walter lippmann (wrote one of the best books ive read on the subject, called Public Opinion), edward bernays (key person in making propaganda be called Public Relations and the person who evolved needing to wanting (in marketing), also a good book of his is Crystallizing Public Opinion). His book propaganda is shit, not worth the 100 or so pages.
