I don't think they're actively making it up. I think a lot of the time they're self diagnosing themselves with disorders due to actual things like anxiety and depression. Similar to how a lot of people self diagnose themselves with Cancer and other life threatening ailments and annoy doctors about it.
Munchhausen syndrome I've heard is more common with women. However there is an appeal for these people to fake disorders largely since it gets them more sympathy. (And Sympathy = power in the furry community). The downside is it causes people with these disorders to feel marginalized and makes people take them significantly less seriously. (There's a reason why very few people with autism who aren't retarded admit to having it. Only the true aspies post it on their profile.)

Autism afaik is something that's diagnosed early in life. If you weren't diagnosed early in life you probably didn't get it. It's not just "I'm socially retarded therefore I'm autistic".

You can go to a doctor and get a diagnosis. Or just having medication is a good indicator it's legit.

Bipolar gets some pretty heavy medications so you tend to need physical evidence of it to prove you're not a drug addict.

It's not a matter of shame or being secretive. It's moreso that a lot of people treat their mental condition like it's fashion. Similar to how people claim to be transsexual or have a different gender.

There's a lot of physical realities to post op life that cause difficulty. Stuff like how hair physically still grows in the "vagina"

It's more likely that there wasn't enough flesh to do the surgery. If the surgeon was able to determine that the vagina would look malformed after the procedure I'm fairly certain they wouldn't go through with it to avoid a potential malpractice suit.
