True, but cloth diapers don't seem to be very popular. If you want to be really technical, most of them would just be running around naked, save for colder climates where animal skins and other things were used.
Even cloth diapers themselves didn't really become a thing until the very late middle ages, safety pins much later, and they were nasty as fuck. Soiled diapers were rarely changed, and when they were, they weren't washed, just hung until the piss and shit dried off.

In medieval scenarios, I like to think that magic is a power so dangerous and unknown that constant practice messes up your body functions and the most common occurrence is incontinence. So magicians had to invent ways to deal with it, because the knowledge of magic is so valuable that pissing and shitting yourself is a small price to pay.

The topic of stories certainly piques my interest. I'm currently writing one (not going to shill it since no one asked), but I'm curious about what people want to read in a Fantasy ABDL story. Trying to find an idea in regards to what a specific audience of degenerates like ourselves desire.
I mean, I've tried to a weird balancing act of setting tone/characterization mixed with massive ABDL sexual under/overtones. But do people want to read a story that is purely MAGICAL REALM or a story that has 'magical realm' elements with a semi-cohesive plot?

I'd say it depends on how long your story is going to be. If it's short (3k-15k words), it can be straight fapfiction, but a semi-coheisve plot won't hurt. If it's above 15k words, you definitely want it to be an actual story, with plot and characters the reader gives a shit about.

Fantasy is such a wide-genre. There's probably more "fantasy" abdl stuff out there than you would think, although that does depend on your definition of fantasy. Is fantastical Halloween stuff considered fantasy? Are magical girls fantasy? How high-fantasy can you go? Is fantasy restricted solely to sword and sworcery and tolkien-esque stuff? What about urban/modern fantasy?

I have an abdl D&D setting. Basically everyone's cursed to be incontinent and damage = mental regression. I've run it for solo players on an rp site a few times and it's an ok prompt I guess. My problem is I'm a much better at DMing in voice vs text or by-post, and any fetish voice chat is really weird to me. I also don't tend to schedule time for my fetishes, so it would be tough to run a regular session.
I've also been in a diaper West Marches group. It's very much like any other WM group, political and driven by egos. .I just wanted to rp without caring about the story and was told to schedule for games or go back to lurking.
On the whole in my experience all fetish D&D is basically just like regular D&D. If there's mechanics for it they aren't present nearly as often as you'd think.

Pretty much.
Typically you all agree on what game or set of rules to use, for example Dungeons & Dragons, and one person is put in charge of running everything.
That person is usually called a Dungeon Master, or DM, or some similar name.
Most games also include a kind of randomization element like dice to keep things interesting.
So you pretend to be someone else in a novel situation that DM describes to you.
Then you tell the DM what you want your character to try to do or say.
Depending on the person or group you may act it out some parts like conversations or just say your character does it.
The DM will then tell you what happens.
If you asked to do something simple the DM might just let it happen and describe the results, but if you wanted to do something complicated the DM will probably ask you to roll the dice to see if you succeed. The DM can ask you roll above or below a certain number, for example, which will depend on how difficult they think the thing you want to do is, making your success more or less likely.
If you ask to do something completely stupid they can also just tell you no, or that rocks fall and everyone dies.
If you're not incredibly autistic you can usually get a feeling for what the DM is trying to do and ask to do reasonable things that make sense in that situation.
If you can't you are That Guy.

In my normal campaign I do discord for voice and roll20 for maps, encounters, and dice rolling. That's be the way to do it. A discord also allows for a downtime rp channel. Voice can be cringe but really all aspects of fetish can be. As with anything in D&D, with the right group it can be fine. Really my biggest hangup about doing a fetish game is that I tend not to schedule exact time for my fetish content. Still I'd be interested in at least lurking in a server if someone sets one up. tbh despite what I said about it doing a West Marches might be the way to go, if there's enough interest.
