The big distinction between Tomo and MouTele is that the former far more blatantly extended the drama, Jun and Tomo literally run from each other at multiple points rather than address the elephant in the room. Because the story is ostensibly about them getting together as a couple.
Moutele has built itself around Nanako actually finding self worth and the strength to accept her gift and herself. Not only that but the series didn't start off with both her and Todo liking each other. To that end it feels far less padded out and Nobel gave a clear time line to a confession in the last couple volumes, so at this point it's more about Nanako passing her exams than anyone avoiding a confession for the 4th or 5th time like in Tomo-chan.
So while both are romcoms in high school, they feel very different. And yes, Mana helps sell the series because she's a top shelf support character and a lot of the personality of the cast with her sass which is much needed with Nanako being so insecure and demure.

Again, since I quit reading around this point I don't know if any new info came up. But from what I saw the plot was set up pretty good to take up this new twist and really shake things up. It seems (to me) like Nanako's telepathy was sort of a two-part deal. She could hear people's thoughts, but she could also sort of receive visions which she assumed were what people were envisioning in their heads. This was really prominent with Toda, whom she assumed was imagining all sorts of lewd stuff. But as she experimented with this a bit with Mana is turns out that she was only getting half-truths this way. I think they did a test with Mana's bra, where it was striped instead of poka-dot or whatever.
Regardless, this really twisted everything back around to Nanako, who up until now was turned off by Toda's "visions". It tied back into the name of the series itself, "妄想" telepathy. Even if she could read minds, she had to start doubting what she heard and saw at this point, because it was either only half true or something she invented with her imagination. I think this was an interesting way to continue the plot, because now instead of just focusing on a cliche romance story, we have Nanako having to confront her preconceived notions of people based off of what she can hear them think.
I guess I typed a lot. What I'm trying to say is that I think her telepathy can be both real and a delusion and the plot could still work. Nanako can read thoughts, but those thoughts are sort of "filtered" through her own head and the result is sort of twisted depending on how she feels or perceives the situation.
