I hate people reccomending this trash like its some second coming of RPGs. Most of the time I ask what was so great about it and I shit you not half of them hadnt even played this shallow trainwreck. I regret the trime I sunk ionto thei shitty game, so obviosuly frontloaded with content that the endgame is the part full of the leftover bugs and sidequests that go nowhere. The followers are two dimensional trannies with one sidequest each that doesnt change or shape them, they still talk exactly the same and repeate the same tiny handful of lines. Combat is fallot 3 tier, and there is little to no variety that even matters when you can just use a plasma carbine to win on the hardest difficulty. Dont even get me started on the ugly world design and oblivion looking character models.
Play a good game instead like Outer Wilds instead of Outer Worlds, pic related

I pirated it a while ago but couldn't even get myself to finish it. Weapons are copy&paste, enemies are copy&paste, environments are tiny and yet still feel empty and lifeless. Story ranges from braindead to mediocre and all choices you can make have little to no effect on the planets that should be completely changed depending on what you do.
All the while the game keeps poking you with lefty nonsense,
including the whole "Ho boy, everyones starving because capitalism" "reveal" that didnt exactly make for a great surprise after you solved several food shortages throughout the story.
All in all it just feels like some small indie project that had no resources to take on a game of this (theoretical)
size so everything had to be downsized and reduced to shit until nothing of value was left.
I mean fuck, this shit makes Todds outdated crap look like some kind of masterwork in comparison.
And yet, the sjws shitheads making this tried to attach it to FNV at every step before launch.
Probably because they knew exactly what kind of lackluster shitheap they had on their hands so they figured pre-orders are the only sales they can get close to full price.

Closest thing I've found is Total War Medieval 2 Cruades which features Byzantium as as major faction, and Crusader Kings 2 which also being a medieval europe sim also has Byzantines somewhere on the map depending on the time period. Total War: Rome II Barbarian Invasion also has the east/west roman empire split if you want to go back a bit. But be careful of what you wish for or you'll get some shit like Assasin's Creed: Istanbul bullshit like they've done with every other critically unexplored historical video game setting.
