Easy assuming you get a good file, time consuming if not. On a scale of PSP to Xbox i'd say its about a Wii
A few things you have to consider

Official ones were made for IDE drives and come with a network port while 3rd pary ones can come with SATA support instead but no network port
You can however mod an official one to support SATA, I recommend this option, some people used to sell diy mod kits for that.You can also buy a ide-sata converter but then have to get a 2.5 inch drive to fit in the ps2

If you already have FMCB installed then its just a matter of getting HD Land OPL and telling OPL to look at the drive for games, and you can use a number of tools to either connect it via network or directly to a computer install games onto the HDD
Doing it without already having fmcb comes with more steps
revive today has an easy to follow guide but the img of FHDB didint fully work for me and neither did the "newbie package" img from one of the playstation mod sites
If the same thing happens to you its still pretty simple
Get Winhiip, and HDD raw copy tool or some tool like it and find an img of FMCB/FHDB to install onto it, its okay if its an older version, you'll just install the latest anyway since the boot time is pretty long compared to installing it on a memory card
format the drive into a ps2 drive with winhiip. make sure its 48bit HDL for bigger drives. Then burn the image onto it with tool of your choice dont worry about games you have to make sure it all works first
get the latest FMCB installer, OPL, HDL. and wLaunchELF and put them in a usb stick, you might not need wLaunchELF but i cant remember I dont think the formatting matters but it might
If things went right then after plugging in your HDD to your ps2, and a long boot time after seeing the FMCB logo you should reach a the ps2 menu,
don't worry if you entered the cd/memory card browser, depending on the version you burned it might not have settings in place to take you to the other menu, just navigate out of the browser to get to the scrolling menu
Plug the usb into the PS2
scroll to find and open uLaunchELF
go to MASS this is the usb in uLaunchELF find and run FMCB installer
Backup your memory stick saves and then install FMCB onto it
Now your boot time isn't absolutely horrid consider getting a second memory card for FMCB and backing up saves in case saving corrupts the first
Now if if the image you burned was an earlier version of fmcb than your installer go ahead and install it onto the HDD aswell,
if not, exit go back to ulaunchelf and place the folder of OPL and HDL where you'd like them, usb, hard drive, or keep it on memory card if you plan to keep it plugged in whenever you're playing
After this go find a guide on creating main screen links with uLE, and change settings to take you to the scrolling menu rather than the cd/mc menu on boot
Go install games with winhiip or whatever other tool you like
Maybe download cover and cd art for OPL, maybe grab some themes for OPL aswell if you're into that
Consider grabbing other ELFs like the nightly builds of OPL or GSM for selecting graphics modes for games
