We usually hear about things getting censored in games, but what about stuff that didn't? What about stuff that got past the censors?
One of the first things that I can think of is a scene from Phantasy Star Zero. In one area of the game, one of your party members, that looks like a loli, is implied to be tentacle raped by a squid monster. What's even crazier, is that you can have your character say they like what they're seeing while it's happening.

These things had next-to-no censorship. Full frontal nudity, tiddy shots, extreme gore, it didn't matter.
Daggerfall is the best example of this. There is zero fucking chance any of this shit would fly today.

PCs were prohibitively expensive for most people until the end of the decade, and much more difficult to work with. They weren't seen as a children accessable platform, but a necessity for people to bring their work home with them. So the assumption was largely that older teens and adults were primarily using it, and developers crafted their games with those age-brackets in mind. Besides, there never was a central authority which had control over the platform, so there was nobody for a ratings agency to hold accountable for ratings. If it was a big title that needed distribution through Babbage's or Walmart - there was potential for them to refuse to stock it. Largely these stores didn't even really know what they had on their hands since it was still a pretty niche market… and mostly it would have been like trying to swat flies by going after individual publishers. If the ESRB could even do anything at all besides wag their finger disapprovingly.
I remember MegaTech games used to be sold at big box retailers for a while before they caught on to what those games actually were.

There were no censors to get past in those days. ESRB didn't even exist until 1994 and it's not like they can force a store not to carry something–it's the stores that operate on their perception of what public opinion is or should be. In the early-mod 90s it was unthinkable to the American public that video games (believed by normalfags to be toys for children) would have sex or violence in them. Then things like Doom and Mortal Kombat became visible. A modestly unpopular RPG wouldn't have been on the radar when real hairy controversies around games like Duke Nukem were still in their infancy. Almost the exact same morality shit happened in the–what was it? Early 2000s? Anyway, it was when American news media and morality crusaders started learning about Japanese porn games like Rapelay. By this time the American video games industry was big enough not to have to kowtow to them outright, but then again it wasn't Americans making that game. Imagine the shitstorm a muckraker could cause if they started to seriously cover the indie porn games scene.

I think that ever since the Gamecube/PS2 era Suda51 and Grasshopper Manufacturer are on a quest to see how much raunchy stuff can they put in their games without having the censors second question them.
In Flower, Sun and Rain (at least going by the american version) there's characters in the mid part of the game that quite literally mock rating systems and people getting offended over smoking, perverts or child abuse in vidya. Sumio even takes some frustration onto the kid and I'm assuming they could do so because the censors couldn't get past how much walking you have to do just in the first few chapters.

That's my biggest problem with modern games. All the AAA companies pretty much own the ESRB, so they could just decide to not give a shit about the proper ratings and just make full-blown sex sims. There's absolutely nothing stopping them, but they keep trying to drag things back and act all about how they "we need to be an example".
