Well in the west it's pretty clear why. As for jap land, I guess the military otaku and regular ones don't overlap very much. Even things like upotte and bullet girls never got hugely popular, I think the most successful one would be girls und panzer.

The only game I can think of is "Order of Eliminate" or something close to that. You play as a cute girl and have to kill a bunch of mutated entities in a metropolitan area. It's made in Unity and has piss, shit, and vomit in it, but that can all be ignored if you try hard enough. Still a lot of trash, but it has a puzzle in it that I have yet to figure out.

Something like a third or first-person tactical shooter that's an in between of ARMA and games like Insurgency or Red Orchestra/Rising Storm. Realistic but not autism levels, lots of customization of both gear and weapons, make it not just a gay multiplayer only game but with a fun singleplayer/co-op mode, and set it in a time frame that isn't just "modern warfare" aka rails rails and deserts, maybe something like totally not Cold War or even totally not WW2 so you can get the best mix of aesthetics and gear.

Because nobody buys them, case in point the bullet girls trilogy; the first game was janky as shit, but it sold enough to warrant a sequel that improved on everything and made full use of both touch screens on top of running at 60fps and the vita's native resolution, and even though the game was fucking perfect it sold like shit, to the point that the third game had to go for the medieval isekai meme, add melee weapons and pander to the soystation 4 owners first (vita got an astoundingly shitty port, even when overclocking it doesn't maintain 30fps, and it looks worse than the second game) even though vitabros were the ones who supported them from the very beginning.

I would like a 3rd person shooter game, no over the shoulder bullshit, kinda plays like conkers bad fur day live and reloaded multiplayer. The maps would have resources, like amour factories, refueling stations, munition storage houses, that could be captured or destroyed. Maps would be fairly Big, you would need a vehicle every now and then to get to where you're going.
You also would not be playing one anime waifu fag girl, but a whole team of them. That you could switch to and back at any time.Characters could be randomly generated then given a random personality with random stats. So that when they do die you'll care for them. Depending on who dies your squad could be driven to fight harder producing military miracles. They shout out their faggy friends name while their states slightly increase. But that doesn't mean your character has to die to trigger a stat increase. Simply seeing their squad leader, or a good comrade in arms, fall from taking a hit could trigger it.
This game would be 3rd person of course, combat would be slightly random also. For instance placing your snipers cross hairs right on the head of another character then pulling the trigger will not always guarantee a kill or a hit. It would kinda be like mech worrier games.
Getting hit in the arms or legs would render that limb useless, you would need your squad medic then. you can also use squad commands.
Sorry to say, but it would be gory, limbs would get blown off, heads would bust open, spilling out contents like a water melon being bash open. You will hear the cries for help, the grunts and moans, the choking on blood, while the squad medic desperately tries to save their wounded fractured lives. You will see blown in half squad member, as she stares blankly towards the sky, awaiting for deaths cold grip to take her.
For every waifu solider kia they will be place in your own memorial along with their info, to remember them by. Also visiting the area of a fallen comrade you may be able to see them from afar. like a ghost basically, depend on certain characters. Some will see a ghost some will become sad, or remember how they died, a flash back maybe.
Soldiers can be captured or executed. Capturing will open up new options you can set up a fee, to have them return to their own squad. Hell pay a captured solider to join your team, though this will be tough, depending on the personality of a captured character. Characters will be marked with a traitor symbol, for potential capture once again. You can choose to give back or abandon or execute.
Abandon soldier's will roam freely seeking work, or a new squad. So this means shes up for grabs. She will have either a fee, or maybe become so desperate she will join freely. So your squad seeing a once team member fighting them will notice, so will the traitor. Depending on personality your traitorous squad member could perform bad on purpose, cause of memory. You could potentially get her back, just think of the possibilities. A traitor solider could live a long time being hired then captured back.
One reason to do all of this is to keep your waifu's from dying, as with everything including war you need funds, this way capturing them would be far beneficial then killing them with the potential to make money. You make money and someone doesn't have to lose a bullet throwing waifu.
As for what era ww1, ww2, coldwar, with experimental hightech weaponry, where resources are scarce. What you will spend money on, vehicles, arms, ammunition, fuel, gear, clothes, food. medical supplies.
Hopefully some talented person creates something similar. I can go into more detail but hell!

It will depend how good you are and what stats your character has. If their is a luck stat vs sharp shooter stat, depending which is higher will have an edge. The higher the stat the higher the percentage of a potential hit or kill. Kinda like marrowinds combat. every shot is a dice roll. But of course a sniper would be far more accurate. Than a rifleman, or a light machine gunner, imagine that sniper having a high luck also high hitting stat, would be far more lethal in combat.

Nobody says the gore has to be ultra-realistic.
I'd go with something simple. You get blood decals on walls and models and a small blood spurt on impact, and the vocalizations are downplayed and increase in intensity from a slight grunt when you get damaged at high health to a yelp at medium health and a short scream at low health. You can also have dismemberment and gibbing, I miss gibbing as long as you stick to a cartoony "everyone is a mannequin full of ketchup" sort of anatomy which is kind of what modern anime does for gore as well.
Graphics, artstyle and the quality of the assets that would go into the game matter a lot as well, but this is already more than most typical modern military shooters do in terms of gore and it's still mild enough to not put most people off but still satisfying enough for people who enjoy gore. Pic related is as graphic as I'd take it.

I noticed that it's a mixture of playing as a boy or a girl for me. The times I've played as a girl is to either get a lesbian scene, the only exceptions are Fallout New Vegas, which I did a couple runs of and changed my character, and Mount & Blade, because I found out the mod I downloaded had a feature where women were treated differently than men.
