Have you considered that the difficulty increase from vanilla may not take into account custom difficulty settings?
Also your inability to hold a city might actually stem from a lack of preparation. Having a noble house buttered up and several hundred thousand denars in store as well as at least 50 Right To Rule in order to start a kangdom are not optional, they are the basic requirement. You absolutely need to have at least 5 lords and a waifu when starting one. Also if you start a kangdom before the 3 year mark on your first go at it you might not have the renown required to field an army sufficientely bloated to hold a town by yourself, that is assuming maxed leadership and all.
If you want a head start heres some powergaymen you can do:
>do not import character, just boost XP rates in the ini
>set lord escape chance to 0 for balance reasons
>start with looting 7
>prioritize ironflesh and powerstrike, then weapon master before all other skills otherwise
>theres hidden chests in Rane and Valonbray that have shit in them at the start of the game, Rane has additional qualis gem, grab the shit ASAP and trade the qualis gem for the elexir at one of the book merchants, thats 6 free attribute points per sip
>always ask the wanderers and slavers in taverns for rumors, wanderes sometimes tell you about new free shit in one of the chests, slavers can sometimes tell you about red brotherhood hideouts (free shit assuming you reach the chest) or sell you some rare units
>the first brotherhood hideout you find will have another qualis gem in its chest, as long as you reach the chest at the end of the valley and grab everything from it, you keep it even if you die afterwards, use that qualis to get another elexir, thats another free 6 attribute points
>join a kangdom as a merc ASAP, and grab the most expensive units you can get from taverns, theyre worth it and the faction you work for pays their wages anyway
>easiest way to earn cash is to ride around with 50 high tier units and gank minor faction bands, assuming looting 7 you can get 2k denars worth of shit from a 20 man heretek party
>start in Ravenstern, the bandits there are the jack of all trades type which means they are master of none and are the easiest to deal with, vanskerry raiders will pelt you with axes, hereteks and snek tits are too heavily armored at the start of the game, singalian raiders will swarm you and ride you down
>hunt bandits for cash, spend cash on the heaviest armor you can buy, then go on a world tour hunting bandits, companions, wanderers and slavers
>from there do whatever
you have to prioritize personal survivability, so heaviest armor and 2 shields is what you want before you can go without
throwing is better early and mid-game, archery is better late-game, crossbows are okay and guns are nieche material throughout the campaign
thats pretty much as optimized of a start I could think of, if you prioritize hunting the 2 free gems thats 12 levels worth of attubute points right there for free
