Let's have a thread about what mods are considered mandatory to play various games.

Pic related has a ton of graphics mods but I don't know about gameplay mods. I never played with the duct tape mod, does it make the game better or worse? It is a signifcant change in design after all.

I know, but it takes me out of games. Real guns have rear and front sights on them unless they're made for extreme close quarters. Even if you can't aim down sights in the game, the guns in game should still have proper sights on them for authenticity. People don't understand the true value of sights and just assume guns are magic.

Overthinked is pretty much a must have for Doom 3. It makes a number of gameplay changes, usually having to do with weapons and ammo, that makes fights and ammo usage more tactical and meaningful. I've tried a number of Doom 3 mods, and surprisingly there really aren't many of them that actually make any meaningful changes to the gameplay or mechanics itself. Overthinked is the only one I found that does it and does it well, most other mods are either graphical or some fan levels/campaigns.

They're pretty balanced to me. It's not like you can just pick an alt fire at random and it will obliterate whatever's in front of you, you need to use the right ones at the right time, just like all the primaries. In any case, those are essentially optional, you can just use the self restriction of never using alts. Even if you consider those a negative, all the other changes made in the mod make it a net plus, and it's probably how Doom 3 should have been when it was shipped. In vanilla, very little is actually challenging and you're swimming in ammo the entire game. Even on nightmare, the "difficulty" of D3 doesn't feel much like difficulty, just an irritation of "don't get hit once" with very little else changed.
