If the NPCs are to be avoided at all costs, what you should focus on is level design. Split the entire level into different stages, the final reward at the last one. Then make transitions between stages that have different costs to pass over.
For instance, you want to ravage a lady while she's taking a bath, so you have the garden of her house with guards outside, the first floor and the second floor, after which you get to her.
The Garden is the first stage, with 2 different ways to pass, either the front door or the back door. The backdoor is a longer route but it has less guards so a simple distraction item would suffice. The front door is more heavily guarded and therefore you'd need
Crossing the transition locks every enemy behind you. Guards outside wouldn't be able to come inside for instance. But being spotted in any stage increases the difficulty of following stages, spawning more guards, moving them more, etc. Fail 2/3 of stages and it's an automatic loss however.
If however you mean that the player is hunting the NPCs and must avoid detection, what you'd go for instead was mind games and proper hunting.
NPCs would see you coming nearby 100% of the time in normal conditions, but your intention is to push them somewhere in particular. The gameplay would be about corraling them somewhere where you can actually grab them.
So if there's 2 doors in a room and you want an NPC to follow through one, you'd make sure to keep it well lit while breaking the lights on the other one, you'd come from one direction to scare it another direction, you'd lay traps before in the direction you're forcing the NPC to follow and you'd cut the lights from it's current location to scare it and make it easier to grab.

Paradox is an actual JRPG. It's something of mix between DQV and FFV and it's pretty big. I think once all three parts are out and translated, it'll be bigger than both of those games combined.

I'd say Paradox is better in this regard, because you can recruit every monstergirl you encounter in the wild and some bosses, progress with them using the right gifts, and unlock consensual non-snuff scenes with them. Though I think there are still some very rare snuff scenes with recruited girls.
