Actually anything from ff qualifies, the most bullshit one i've seen is the danjuro in vanilla ffxii, 1% droprate on the lowest floors of toughest dungeon in the game (which by the way has no map or minimap) and only if you have a chain of 500 kills on those acid worms, which means killing worms over and over again without picking up their loot (do it by mistake and it's back to square one) and changing screens constantly, the fucking dagger isn't even good since it has mediocre attack (by the time you can get it you can also buy a stronger one in every shop), no element or status and daggers are shit anyway, to top it off the best weapons in the game (zodiac spear, tournesol, formalhaut, artemis bow) can be gotten just by not opening certain chests, backtracking or killing dozens of some pretty basic enemies (plus 5-6 moon wyrms, easily done with formalhaut) for their drops.

Heavenly Onyx Cloud Serpent in WoW. Didn't drop until the final patch of Mists of Pandaria when the boss was up since launch. It had/has a 1/5000ish drop rate. Some folks say 1/2000, this is incorrect. The game first rolls to see if you get loot, if you do it rolls to see what on the drop list. The mount itself is insanely rare on the list but the gear is all insanely common, so if you get an item it will probably be the shitty old pvp set gear. You can only kill the boss once a week per character. There are individuals who run it with tons of characters who report not seeing a drop even in the 8000s of attempts.
When WoD was launching WoW released an info graphic of stats and due to the bug, they had to admit the mount was rarer than even the Black Qiraji War Tank which only a handful of people per server could have.

Back when MMO's were still a thing, in Star Wars Galaxies you could get a set of Mandolorian armor if you were willing to undergo a retardedly difficult unmarked quest.
First you had to kill a bunch of Blacksun mercenaries that were usually above max level, and hope they dropped a schematic for an individual armor part.There was one schematic per part, and they dropped randomly.
Then you had to recruit an armorsmith you could trust of a high enough level, take them to what was the most dangerous dungeon in the game, the Death Watch Bunker, and then protect them while they crafted your armor, hopefully not screwing it up because the crafting process could fail, costing you the schematic and whatever resources were in play, all the while constantly fighting off mobs of high level bullet sponges.
For a suit of armor that was rare, but not much that much better than some crafted armors, which could be destroyed, and the quality of which was dependent on the person who crafted it.
Who could also decide to fuck you over and not give it to you when they were done with it, or decide to charge you a fuckton of money for it.

In PSO the name you give your character when you make it determines what rare items can drop and from what. This is called a section ID. Each ID can get only a fairly small portion of the rare items in the game to drop. You have to rely on characters with other section IDs for items that do not. The game also has very limited character slots so you can't have characters with every section ID at a time.

The rarest item I know of is the mirror of kalandra in Path of Exile. The drop rate is so low you can play for thousands of hours and never have one actually drop for you. The drop rate supposedly gets changed back and forth without any patch notes, but I think I saw somewhere on the PoE forum that the highest estimated drop rate it's ever been was something like 1 in 10,000,000. Sadly so few drop despite 50,000+ peak players playing at the same time that it's too hard to be accurate(one drops every few days for somebody). Note that it's a currency item that can make a copy of any armor/weapon so it actually is pretty useful and does get consumed for the highest tier equipment.
